Please bring them the day of the appointment, it is required by the health department.​​
1. The clients (child's) birth certificate
2. A photo ID of the client (child); i.e. school ID, state ID, passport, school yearbook.
3. A photo ID of the parent/guardian: must be listed on the birth certificate / guardianship paperwork (no exceptions)

We prefer you to make an appointment, as walk-in availability is very limited. You can always stop in for a walk-in, but appointments take priority. We want you to get the best service possible without the wait.

Please avoid calling ahead to see if there is walk-in availability. We may see a bit of space in the calendar, but by the time you get here there may not be enough time. We cannot predict that, and that can be really frustrating for you to drive here only to be told there's no longer time available. Let's avoid that by making appointments or just stopping on in!
Appointments require a $30 deposit to book, which is subtracted from the total at the time of the appointment. There are no refunds for missed appointments or cancellations less than 24hours in advance. We appreciate your understanding!
