Required paperwork for minors.
(ages 5yr - 17yr)

Please bring them the day of the appointment, it is required by the health department.​​

1. The clients (child's) birth certificate

2. A photo ID of the client (child); i.e. school ID, state ID, passport, school yearbook.

3. A photo ID of the parent/guardian: must be listed on the birth certificate / guardianship paperwork (no exceptions)
